(op\u00e7\u00f5es atuais)<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c0b4973 elementor-widget__width-initial elementor-widget elementor-widget-text-editor\" data-id=\"c0b4973\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><strong>Na maioria dos casos, n\u00e3o \u00e9 necess\u00e1rio tratar.<\/strong>\u00a0As op\u00e7\u00f5es dependem do\u00a0<strong>contexto cl\u00ednico<\/strong>:<\/p><p>\u00a0<\/p><p>\u00a0<\/p><ul><li><strong>Medidas gerais para todos:<\/strong><ul><li><strong>Controle de fatores de risco vascular<\/strong>(press\u00e3o, diabetes, colesterol),\u00a0<strong>n\u00e3o fumar<\/strong>, atividade f\u00edsica e alimenta\u00e7\u00e3o cardioprotetora.<\/li><li>Em viagens longas\/condi\u00e7\u00f5es de risco para <strong>trombose venosa<\/strong>, medidas de preven\u00e7\u00e3o (hidrata\u00e7\u00e3o, mobilidade; anticoagula\u00e7\u00e3o quando indicada por outro motivo).<\/li><\/ul><\/li><li><strong>Ap\u00f3s AVC isqu\u00eamico\/TIA sem causa identificada (criptog\u00eanico), em adultos selecionados<\/strong>:<ul><li><strong>Fechamento percut\u00e2neo do FOP<\/strong>(dispositivo por\u00a0<strong>cateter na virilha<\/strong>)\u00a0<strong>pode reduzir recorr\u00eancia de AVC <\/strong>quando comparado ao tratamento medicamentoso isolado,\u00a0<strong>ap\u00f3s investiga\u00e7\u00e3o completa<\/strong>\u00a0para excluir outras causas (como\u00a0<strong>fibrila\u00e7\u00e3o atrial<\/strong>\u00a0e\u00a0<strong>doen\u00e7as das art\u00e9rias<\/strong>).<\/li><li>A decis\u00e3o \u00e9 do <strong>Heart\u2013Brain Team<\/strong>\u00a0(cardiologia, neurologia, hemodin\u00e2mica, cirurgia), considerando\u00a0<strong>idade<\/strong>,\u00a0<strong>anatomia do FOP<\/strong>,\u00a0<strong>tamanho do shunt<\/strong>,\u00a0<strong>aneurisma do septo<\/strong>,\u00a0<strong>fatores tromb\u00f3ticos<\/strong>\u00a0e\u00a0<strong>prefer\u00eancias do paciente<\/strong>.<\/li><li>Se <strong>n\u00e3o houver fechamento<\/strong>, utiliza-se\u00a0<strong>antiplaquet\u00e1rio<\/strong>\u00a0(p. ex., AAS) ou, em situa\u00e7\u00f5es espec\u00edficas (trombose venosa, trombofilia),\u00a0<strong>anticoagulante<\/strong>\u00a0\u2014 decis\u00e3o individual.<\/li><\/ul><\/li><li><strong>Indica\u00e7\u00f5es especiais (casos selecionados):<\/strong><ul><li><strong>Doen\u00e7a descompressiva recorrente em mergulhadores<\/strong>, apesar de medidas preventivas.<\/li><li><strong>Platipneia-ortode\u00f3xia <\/strong>com evid\u00eancia de shunt significativo.<\/li><\/ul><\/li><\/ul><div>\u00a0<\/div><p><strong>Enxaqueca com aura:<\/strong>\u00a0a\u00a0<strong>oclus\u00e3o do FOP n\u00e3o \u00e9 tratamento padr\u00e3o<\/strong>. de\u00a0<strong>antiplaquet\u00e1rio<\/strong>\u00a0(ou\u00a0<strong>anticoagulante<\/strong>\u00a0quando indicado) e acompanhamento com\u00a0<strong>neurologia\/cardiologia<\/strong>.<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0928680 elementor-widget elementor-widget-heading\" fechamento percut\u00e2neo<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-bc4e759 elementor-widget__width-initial elementor-widget elementor-widget-text-editor\" data-id=\"bc4e759\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<ul><li><strong>Recupera\u00e7\u00e3o<\/strong>: alta precoce; evitar esfor\u00e7o pesado por alguns dias.<\/li><li><strong>Medica\u00e7\u00f5es<\/strong>: costuma-se usar <strong>dupla antiagrega\u00e7\u00e3o<\/strong>\u00a0por um per\u00edodo curto (geralmente\u00a0<strong>1\u20136 meses<\/strong>, conforme o dispositivo\/protocolo) e depois\u00a0<strong>um antiagregante<\/strong>\u00a0a longo prazo (dura\u00e7\u00e3o individual).<\/li><li><strong>Riscos<\/strong>:\u00a0<strong>fibrila\u00e7\u00e3o atrial<\/strong>\u00a0pode ocorrer nas\u00a0<strong>primeiras semanas\/meses<\/strong>\u00a0(geralmente transit\u00f3ria);\u00a0<strong>trombo no dispositivo<\/strong>\u00a0\u00e9 raro (reduzido com a antiagrega\u00e7\u00e3o); shunt residual pode ocorrer e costuma ser pequeno.<\/li><li><strong>Endocardite<\/strong>: recomenda-se<strong>profilaxia antibi\u00f3tica apenas nos primeiros 6 meses<\/strong>\u00a0(at\u00e9 \u201cendotelizar\u201d) ou se
